Changes in the way a person speaks could be an early warning sign of cognitive decline, according to a large new study that found voice disorders may be more strongly linked to future dementia risk than hearing loss alone. Researchers analyzed data from 833,417 patients, tracking people after they were diagnosed with either a voice disorder, hearing loss, both conditions, or neither. They then examined who later developed cognitive impairment or dementia.
The findings revealed a striking pattern: people with a diagnosed voice disorder but no hearing loss were 58 percent more likely to develop cognitive impairment or dementia than people with neither condition. By comparison, those with hearing loss alone faced a 27 percent higher risk. The strongest association was seen among people who experienced both a voice disorder and hearing loss.
Voice disorders included symptoms such as changes in pitch, reduced speech clarity, altered volume, reduced vocal range, pain when speaking or singing, and other medically diagnosed vocal conditions. The research builds on previous evidence linking hearing loss and dementia. Hearing loss is already considered the largest modifiable risk factor for dementia, but the new findings suggest voice problems may serve as an even earlier indicator in some cases.
Robert Sataloff, the study's senior author, said reduced communication caused by voice disorders could play a role. The researchers noted that social interaction and mentally stimulating activities are known to support cognitive health. If speaking becomes uncomfortable or difficult, people may gradually withdraw from conversations, group activities, or hobbies that help keep the brain engaged.
Sataloff also pointed to Parkinson's disease as an example of how voice changes can emerge before more obvious symptoms appear, raising the possibility that vocal changes could serve as an early neurological warning sign in some people. Chandril Chugh, a neurologist who was not involved in the research, told Newsweek that the connection between voice and cognition is biologically plausible because both depend on many of the same neural systems. He explained that these regions are often affected early in neurodegenerative diseases, which may allow subtle vocal changes to emerge before memory symptoms become noticeable.
According to Chugh, the voice acts as an ongoing reflection of complex brain function. The findings could eventually have practical implications for screening, particularly because voice monitoring is relatively simple and inexpensive. However, experts caution that vocal changes alone should not be viewed as proof that someone is developing dementia.
